if the facility is certified to provide a specialized program, will also review resident E-scores. |

Licensees who have participated in the Focused Onsite Renewal Inspection are required to complete: |

|
Once a facility has completed a Focused Onsite Renewal Inspection and their license has been
successfully renewed, it is guaranteed that all future renewal inspections will be conducted
as a Focused Onsite Renewal Inspection. |
